I acknowledge that bicycle riding carries with it the potential for property loss, serious injury and even death. The risks include, but are not limited to, those caused by vehicular traffic, actions of other people, including other bike riders or pedestrians.

I understand the Cruzbike is a front-wheel drive bicycle that may feel awkward or wobbly while I am learning to ride it. I understand that I may fall and get seriously hurt, or accidentally swerve in front of a car and be killed. I hereby assume all of the risks of riding or test-riding a Cruzbike, including injury related to improper adjustment of brakes, pedals, handlebars, or other parts: adjustments that I take full responsibility for setting and checking before I ride.

I certify that I am physically fit to test ride a Cruzbike and that I have not been advised otherwise by a qualified medical person.

I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows: Waive, Release and Discharge from any and all liability for my death, disability, personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me from riding or test riding this bicycle THE FOLLOWING ENTITIES OR PERSONS:

(A) Cruzbike, Inc., their directors, officers, employees, volunteers, representatives, and agents; (B) if the test ride is conducted in conjunction with an organized event; the event holders, event sponsors, event directors, or event volunteers.

I hereby consent to receive medical treatment, which may be deemed advisable in the event of injury, accident and/or illness during this ride or event.

This Accident Waiver and Release of Liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
